This session will review emerging research on the pharmacology of methadone and buprenorphine during pregnancy and discuss how physiologic changes in pregnancy influence medication exposure, dosing needs, and treatment outcomes. We will highlight findings from recent clinical and pharmacokinetic studies and translate these results into practical approaches for optimizing medication treatment for opioid use disorder during pregnancy. Additionally, pharmacokinetic studies and emerging safety data for HCV treatment in pregnancy will be reviewed. • Describe physiologic changes during pregnancy that influence drug metabolism. • Summarize evidence and clinical considerations for initiating methadone or buprenorphine treatment for opioid use disorder during pregnancy in the fentanyl era, including medication selection, safety considerations, and barriers to treatment initiation. • Apply current evidence to optimize continuation of medication treatment during pregnancy, including considerations for dose adjustments, split dosing strategies to managing withdrawal symptoms and cravings. • Review evidence on maternal, postpartum, and neonatal outcomes associated with methadone and buprenorphine treatment, including neonatal opioid withdrawal syndrome and considerations for medication management in the postpartum period. • Present the most recent safety and efficacy data on HCV treatment in pregnancy and practical recommendations for treatment providers
