This session examines innovative nurse-led models of care within prisons and beyond, focusing on best practices in TB/BBV management, harm reduction, and peer-led support. Drawing on insights from multiple countries, we will examine strategies to enhance continuity of care, minimize loss to follow-up, and foster stronger collaboration between health and justice systems to improve long-term health outcomes for individuals transitioning back into the community
